Here you will find standard school forms that can be useful from time to time. Please remember to keep Arbor’s Parent Portal up to date with latest information too. The forms below can be returned to the School Office by hand, or sent in after contact with the School Office. Food Related

  • Caterlink food allergy and intolerance form. Use this form to update the school catering contractor about food allergies and intolerances. This must be done in addition to notifying the school through admissions and updating processes. For the purpose of completing this form, the school’s Unit Number is 8687.

Medical Related

Allergy Action Plans

These Allergy Action Plans are for children at risk of anaphylaxis. They have been designed to facilitate first aid treatment of anaphylaxis, to be delivered by people without any special medical training nor equipment apart from access to an adrenaline autoinjector (AAI). The plans are designed to function as Individual Healthcare Plans for children with food allergies, providing medical and parental consent for schools to administer medicines in the event of an allergic reaction. This includes consent to administer a “spare” back-up adrenaline auto-injector pen. Access the Department of Health guidance on the use of AAIs in schools here: www.sparepensinschools.uk.

These Action Plans are for completion by a healthcare professional, for example GP, and not Teachers and/or parents.

Asthma Action Plans

Asthma Action plans are required from a healthcare professional (such as a GP, allergy specialist, or paediatrician) for all pupils with Asthma.
