Here you will find standard school forms that can be useful from time to time. Please remember to keep Arbor’s Parent Portal up to date with latest information too. The forms below can be returned to the School Office by hand, or sent in after contact with the School Office. General Admissions Information
Please keep your pupil, parent/carer, medical and emergency contact information up to date.
Parents and carers should regularly login and review pupil profiles in our Management Information System’s parent portal, which you can access by clicking here.
School Dinners
All allergies, medical information including intolerances should be recorded on your child’s pupil profile using the Parent Portal link above.
Please also provide relevant information via Caterlink’s food allergy and intolerance form, which is our catering provider’s form. For the purpose of completing this form, the school’s Unit Number is 8687.
Free School Meals (Pupil Premium) Eligibility
Your child may be entitled to Free School Meals and the school may be entitled to additional funding (known as Pupil Premium).
The school can check this for all parents who complete the short Registration for Free School Meals (FSM) Eligibility (Pupil Premium) form, click here.
You child may be eligible if you (parent/carer) receive:
Universal Credit and have an annual household income of £7,400 or less
Support under part VI of the Immigration and Asylum Act 1999
Income-related Employment and Support Allowance
The guaranteed element of Pension Credit
OR
Universal Credit and have an annual household income of over £7,400
Your child might also qualify if you have no recourse to public funds (NRPF).
If in doubt, please complete the form and the school will run the check.
Medical Related
Medication Administration Form. Use this form if your child requires medication during the school day.
Allergy/Asthma Action Plans
Parents and carers of all pupils who have a known allergy to a food or insect sting, and all pupils with asthma should have been provided with an allergy/asthma action plan from a healthcare professional. Please provide a copy of this to school.
If your healthcare professional does not have their own template, please use the templates below, which will still need signing by a healthcare professional. These Action Plans are for completion by a healthcare professional, for example GP, and not Teachers and/or parents.
The Allergy Action Plans below are for children at risk of anaphylaxis. They have been designed to facilitate first aid treatment of anaphylaxis, to be delivered by people without any special medical training nor equipment apart from access to an adrenaline autoinjector (AAI).
The plans are designed to function in part as Individual Healthcare Plans for children with food allergies, providing medical and parental consent for schools to administer medicines in the event of an allergic reaction. This includes consent to administer a “spare” back-up adrenaline auto-injector pen.
